Department
4C SouthOrthopedics/Urology Medical/Surgical Unit (4C South) specializes in orthopedic trauma and spinal cord injury patient care. Nurses in the orthopedic med/surg unit focus on each patient as an individual, working cohesively as a team to achieve the best patient outcomes. This 17-bed med/surge unit is fast-paced with a strong team-focused culture. The nurse-to-patient ration is 1:5 and CNAs have an outstanding patient ratio of 1:5. Individuals who are highly ambitious and hardworking thrive in this team-oriented environment.
Job Summary
Telemetry Technician: Operates telemetry and hospital computer-based equipment and comprehends the capabilities of the cardiac monitoring system. Reports any unusual activity to nurse or charge nurse and documents communications according to the clinical care guideline. Manages equipment appropriately, providing prompt communication to the nurse leadership team. CNA: When working in CNA role, under close supervision, assists professional nursing and care providers in providing care to all patients with respect for their uniqueness. Collaborates with the RN and healthcare team. Assists with basic patient care and activities of daily living. Maintains unit specific documentation as required. Orders, maintains and utilizes supplies for the assigned unit. Demonstrates the equivalent skills and competencies of the CNA.
Essential Functions:
Telemetry Technician General Operations:
Provides observation of cardiac monitors to maintain consistent knowledge of any changes in monitor patterns and potential lethal or dangerous dysrhythmias. Does not leave monitors unattended.
Promptly notifies unit charge nurse or delegee of cardiac rhythm and/or pulse oximetry changes.
Assists staff with reviewing monitor data, as needed.
Documents the interpretation and imports rhythm strips in patient's electronic medical record per unit policy, and more frequently if ordered or directed by nurse leadership.
Admit patients to the monitoring system, sets alarms per nursing staff directions, as per provider order,
Acts as a liaison among all units that provide telemetry and continuous pulse oximetry monitoring.
Provides data and interpretation support in critical situations.
Mentors and orients others as directed by unit leadership.
Maintains a clean work environment. (10%)
Data Collection and Documentation:
Observes, gathers, interprets, and records telemetry recordings, reporting cardiac rhythm changes and information on monitored patients.
Documents strip interpretations and imports strips into patient's electronic medical record. Verifies patient identity with admissions, discharges and strip imports
Gives report to oncoming shift, per established standard work.
Confirms strips are importing in the electronic medical record. (10%)
CNA General Operations:
Obtains vital signs and differentiates normal from abnormal findings. Reports abnormal values or conditions to the RN/LPN. (EX. Abnormal VS, bleeding, safety concerns, family issues.)
Assists the RN/LPN in carrying out the established plan of care for assigned patients.
Functions appropriately in critical situations.
Participates as a member of the patient care team by asking for additional tasks, or helping others when not busy.
Accurately gathers information and performs data entry. (10%)
Documentation:
Documents Plan of Care making sure information is properly transcribed and complete.
Adheres to the hospital's principles and practices of Informing Clinical Practice, Interconnecting Stakeholders, Improving Population Health, and Personalizing Care. (10%)
Data Collection:
Collects clinical data to contribute to the Plan of Care in conjunction with the RN/LPN.
Performs chart audits to promote quality improvement.
Reports response of patient/family or changing condition to the RN/LPN and helps to appropriately update the plan of care.
Participates in practice changes related to nursing research. (10%)
Implementation/Outcomes:
In coordination with the RN/LPN, implements care based on prioritized patient needs.
Participates in a unique, outcome focused plan of care based on current nursing practice with measurable outcomes and goals for the patient/family. (10%)
Collaboration:
Participates with the Coordination of care with multidisciplinary team.
Participates in the discharge planning process.
Assists the RN/LPN to identify realistic expected outcomes to facilitate the patient's optimum level of health as well as the anticipated continuation of care.
Works with the RN/LPN to provide a unique, outcome focused plan of care with measurable outcomes and goals for a patient/family.
Interacts with colleagues and the public in a manner consistent with the standards set forth in Denver Health's Code of Conduct, and behavior standards outlined in Trust, Respect and Excellence.
Participates in the peer and coworker review process. (10%)
Safety/Quality:
Practices sound safety and infection control practices according to hospital policies and procedures; Identifies safety and infection control issues.
Reports quality, effectiveness and efficiency of nursing care provided.
Performs qualify control on appropriate systems and equipment.
Promotes quality care based on going monitoring of patient/family response or changing condition.
Provides a safe environment for all patients. Assuring proper patient positioning and patient care throughout the patients stay.
Ensures practice and adheres to professional standards, statutes, regulations, hospital and departmental policies, and standards of care.
Assumes accountability for achieving goals set jointly with the manager, educator, and charge RN for the next evaluation period. (10%)

Acts as a patient advocate.
Assures patient privacy before, during and after procedures.

Patient Care Positions:

Education:
Post High School Diploma or Certificate Program Required
Work Experience:
Typically, one year experience in a health care/telemetry setting providing telemetry service required
Licenses:
CNA-Certified Nurse Aide - ANCC - American Nurses Credentialing Center required
BLS-Basic Life Support (BLS/CPR) - AHA - American Heart Association required
Knowledge, Skills and Abilities:
Knowledge of and the ability to use medical equipment and instruments to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment.
Effective skills in communication, problem solving, interpersonal relationships and prioritization. Skill in maintaining records and recording test results. Skill in establishing and maintaining effective working relationships with patients, employees, physicians and the public.
Ability to maintain quality control standards. Ability to react calmly and effectively in emergency situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly. Ability to communicate effectively with peers and understand directions.
